I hereby authorize the repair of my vehicle, I agree that H&H PDR Services (H) is not responsible for lost or damage to this vehicle and/or articles left in the vehicle due to fire, theft or any other cause beyond its control. I also hereby grant permission to this company’s employees to operate the vehicle for the purpose of transporting to and from our shop, testing and/or inspection. Further, I acknowledge that if closer analysis reveals additional repairs, I authorize (H) to manufacture access to damage that may not be otherwise accessible due to their location on the vehicle. I acknowledge that parts fail on occasion and that repairs done by H&H are purely cosmetic and noninvasive and, therefore, in no way affects the mechanics of my vehicle. I am aware and agree that (H) reserves the right to repair any parts written for replacement on the corresponding claim. Replaced parts will be disposed of unless otherwise instructed. I acknowledge that (H) is not responsible for prior and/or unrelated damage(s) included in but not limited to damage(s) noted on vehicle diagram or in “notes” section of this document. I understand that additional unrelated, prior damage may be found during the repair process under closer examination of my vehicle, and that I will be notified of any new discovery.
The total amount of the repair charges must be paid before the attached vehicle can be released for delivery. If insurance coverage pays either a portion of or the total amount due, I acknowledge that the insurance check/draft must be obtained by myself or sent in advance by the insurance company. I also acknowledge that I must make arrangements with any lien holders or other payees to endorse the insurance check/draft prior to the release of the repaired vehicle.
I authorize (H) and its representatives to communicate with my insurance company, adjusters, appraisal companies, and third-party administrators regarding: filing of claim, damage documentation, repair estimates, supplemental estimates, repair procedures, inspection scheduling, parts pricing and labor operations. I further authorize all payments and supplements to be made payable directly to (H) for the consideration of repairs made to the vehicle. I hereby authorize (H) to act as power of attorney to sign for or endorse any checks and/or drafts made payable to me and any release thereto, as settlement for my claim for damage to this vehicle. To secure payment in the amount of the repairs, an expressed mechanic’s lien on the vehicle is acknowledged, and I further agree to pay reasonable attorney’s fees and costs in the event that legal action becomes necessary to enforce this contract.
